Job Title: Social Care Facilitator

Location: Walsall

Service Area: Adult Social Care

Contract Length: 3 months (with potential for extension)

Working Structure: Hybrid - minimum two days in office per week, operating on a seven-day rota with two rest days

Hourly Rate: £18 per hour

Requirement: Full UK driving licence

Essential Experience - Applicants must have demonstrable experience in:

• Conducting assessments
• Managing caseloads
• Delivering support within community settings

Role: We are seeking a Social Care Facilitator to join our Adult Social Care team in Walsall. This role involves delivering a personalised Intermediate Care Service to individuals with a variety of needs, helping them regain independence and confidence following hospital discharge, illness, crisis, or changes in personal circumstances.

Key Responsibilities

• Deliver tailored support to individuals recovering from illness or crisis, enabling them to rebuild skills and confidence
• Coordinate and implement active Reablement programmes in line with individualised plans
• Work collaboratively within a multidisciplinary team to promote independence and reduce reliance on long-term care
• Support individuals in accessing community resources, building informal support networks, and participating fully in society
