We have a great opportunity for you to be able to make a difference to your local community.

We are seeking Reablement Support Workers (RSW) to join Suffolk County Council's teams in locations across Suffolk.

We offer support plans to help people reach individual goals that mean they can confidently live at home. Help can last up to six weeks but may end sooner if the individual reaches their goals before then.
As a Reablement Support Worker you will play an important role in helping people in your local community stay independent in their own homes for as long as possible by providing short term care and support.
The aim of the support is to:

• Prevent hospital trips that are not needed.
• Help people in their home after a hospital stay.
• Avoid people from moving into a care home before it's needed.

This is a really rewarding role where you will be able to make a real difference by helping people regain the independence to continue doing the things they love. Your role will be to support your customers with daily tasks which they may be facing temporary difficulty with such as cooking, moving around the house, or bathing, and provide the help and guidance needed for them to get back to living independently.
Throughout the 6-week support plan you will see the impact of your care and support as your customers gradually require less help in these tasks.

You will need to be self-motivated, caring and passionate about offering support. You will work with a variety of individuals and so excellent communication skills and an ability to easily develop rapport play an important part of the role to help you offer the best support.

As this role requires travel between different locations you must hold a full and current driving licence and have access to personal transport.

Ipswich & surrounding:

• Saxmundham , Woodbridge, Felixstowe. Eye.

Bury St Edmunds & surrounding:

• Bury Rural, Bury Town, Sudbury, Mildenhall, Newmarket

North Suffolk:

• Waveney, Lowestoft North
