Social Care Facilitator
Pay Rate: £18ph PAYE / £22.91 Umbrella
Working Arrangements: Civic centre based. Office working 2 days a week. Requirements: Driving licence

Opus people solutions are currently recruiting for a Social Care Facilitator for our client based in Walsall on a temporary on-going basis for 3 months.

- Undertake Care Act 2014 duties, including Section 9 Needs Assessments and Section 10 Carer's Assessments.
- Complete reviews and develop person-centred support plans to meet identified needs.
- Support adults to access appropriate services, community resources, and preventative interventions.
- Work in partnership with individuals, carers, families, and multi-agency professionals to achieve positive outcomes.
- Manage a caseload, maintaining accurate records and ensuring statutory responsibilities are met.
- Experience working with adults with learning disabilities is desirable.
- Strong assessment, communication, and care coordination skills are essential.

This role is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975. As this role is eligible for an Enhanced DBS check, applicants are required to declare any cautions and convictions that are not protected under current filtering rules
