Key responsibilities will include:

  • Delivering comprehensive Occupational Therapy assessments with a strong person-centred and strengths-based approach.
  • Applying sound clinical knowledge of physical health conditions to inform assessment, intervention, and support planning.
  • Managing and prioritising a diverse caseload independently, seeking supervision and support where appropriate.
  • Assessing for and arranging minor equipment, specialist equipment, and major adaptations, including Disabled Facilities Grants (DFGs).
  • Completing moving and handling assessments and providing specialist guidance and recommendations.
  • Contributing to rehousing assessments and reports where required.
  • Signposting individuals and carers to appropriate support services and community resources.
  • Maintaining accurate, timely, and comprehensive case records and documentation.
  • Working collaboratively with individuals, carers, colleagues, and partner agencies to achieve the best possible outcomes.
  • Applying a strong understanding of safeguarding principles, the Care Act, and relevant legislation within practice.
