Personal Advisor - Leaving Care Team

Pay Rate: £19.32 PAYE / £24.59 Umbrella

Working arrangements: Mon to Thur 8:45 - 17:00, Friday 8.45 to 16:00

Length of Assignment: 12 weeks with possible extension

Additional Requirements : Enhanced DBS with Adults & Children's Barred List / Driver

About the Role:

As a Personal Advisor within the Leaving Care Service, you will manage a complex diverse caseload of young people and support care leavers, including former and current asylum seeking children/young people, those with an unresolved immigration status and young people with disabilities. You will undertake needs assessments and develop plans that respond to the identified needs of the care leaver, supporting independence and the transition to adulthood. Working with a range of agencies and professionals, you will promote good outcomes for young people and build enduring support networks based on trust and confidence.

Key Responsibilities:

- Manage a complex diverse caseload of young people.

- Work with service users to achieve identified goals.

- Undertake needs assessments and develop plans that respond to the identified needs of the care leaver.

- Produce pathway plans in line with procedures and statutory guidance, ensuring they are SMART and outcome focused, and focus on achieving independence and the transition to adulthood.

- Work with a range of agencies and professionals as to care planning for young people to promote good outcomes.

- Work with professionals to ensure service delivery and design meets current best practice and meets the need of the community.

- Build enduring support networks for the care leaver and build working relationships with the care leaver based on trust and confidence.

- Understand and respond to the individual needs of the care leaver having regard to their diversity.

- Encourage care leavers to participate in decision making and in the wider development of the leaving care service.

- Work independently with the care leaver and their family.

- Produce clear and concise reports.

- Use tools and discussions with manager and team members to support with decision making.

- Be accountable for own personal development in relation to the professional role and boundaries; and undertake training.
