We are seeking an experienced and committed practitioner to join our Adult Social Care team. The role focuses on delivering high‑quality statutory social work and ensuring adults receive the right support at the right time.
Key Responsibilities
- Care Act Assessments: Complete strengths‑based Care Act eligibility assessments to determine needs and outcomes.
- Reviews: Undertake timely reviews of existing care and support plans to ensure they remain appropriate, safe, and effective.
- Care & Support Planning: Develop person‑centred, outcome‑focused support plans that promote independence, wellbeing, and choice.
- Safeguarding Enquiries: Carry out Section 42 safeguarding enquiries, working closely with partners to ensure adults at risk are protected.
- Mental Capacity Assessments: Undertake MCA assessments in line with statutory guidance to support best‑interest decision‑making.
- Maintain clear, accurate case records and work within legislative and local authority frameworks.
Requirements
- Experience in adult social care, including Care Act work.
- Strong assessment and analytical skills.
- Knowledge of safeguarding procedures and MCA/Best Interests.
- Ability to work independently and manage a varied caseload.
- Excellent communication and recording skills.
