About the job:

We have an exciting opportunity for an experienced and qualified Social Worker to join Suffolk County Council’s Adult & Community Service (ACS). The successful candidate will work in an Integrated Neighbourhood Team to provide support and advice around reablement and personalisation to its customers, to enable them to remain as independent and safe as possible .

  • Work with customers to identify and deliver personalised good quality outcomes to the highest standards,
  • Play a key role in promoting personalisation, independence, well-being and choice by carrying out complex assessments, enablement plans, support plans and safeguarding plans under the Care Act 2014,
  • Offer on going advice in line with the principles of Supporting Lives Connecting Communities – promoting self-support, reablement and personalisation,
  • Work with and support a wide range of people whose situations are complex and challenging and who are vulnerable or disadvantaged in some way, including Adult Safeguarding work and mental capacity work,
  • Work closely with a wide range of internal and external partners to identify services appropriate to the individual and ensure all parties are engaged throughout the support planning and delivery of services.
  • Experience working with Adult Safeguarding,
  • Knowledge of the Mental Capacity Act 2005 and the Care Act 2014 and ability to apply it,
  • Ability to work with adults with complex needs, are disadvantages or vulnerable,
  • Ability to promote independence,
  • Effective communication skills,
  • Ability to work effectively as part of a team,
  • Flexible approach to work.
  • Candidates must have a minimum of 2 years post-qualifying experience in adult’s services,
  • Social Worker qualification at degree level or equivalent,
  • Evidence of continued professional development,
  • HCPC registration,
  • Current enhanced DBS certificate or willingness for a DBS check to be carried out.
  • £400 ‘Golden Hello’ payment
  • Holiday entitlement of up to 31 days per annum, pro rata (including bank holidays)
  • 3% pension contribution from employer
