About the job:

We require an Occupational Therapist to join the Adult and Community Services team. You will work as part of a team which is responsible for the social care provision to the Ipswich and East area.

The team will provide a range of timely information, support and advice around reablement and personalisation to it’s customers, to enable them to remain as independent and safe as possible.

You will also work with customers and their carers, whose needs are very complex, to assist them with their plan of support. You will work closely with colleagues from Home First, Independent Providers, Health, the Boroughs and Districts, the Multi Agency Safeguarding Hub, and a wide range of agencies such as those in the voluntary sector.

  • To manage a complex caseload, prioritising work, allocating cases and time management, including interpretation of FACs with regard to OT related issues
  • To carry out background research on referrals
  • To complete asset based face to face assessments with customers and carers to address functional needs and where appropriate care issues, taking into account available resources
  • To record assessments, notes and recommendations onto databases and accurately recording outcomes
  • To develop, implement and review STEP plans with customers, setting and reviewing goals for reablement
  • To act as an Assistive Technology ‘champion’ within teams
  • To make specific recommendations by completing written applications and specifications for Disabled Facilities Grant (DFG), Community Equipment Service (CES) and writing letters of recommendation to Registered Social Landlords and Borough and District Councils.
  • Making referrals to Community Health and partner agencies e.g. Age UK, Red Cross, Suffolk Careline
  • To support and oversee recommendations by OTAs and CCPs
  • To contribute to the team systems for responding to urgent work
  • To participate effectively in supervision, team meetings and professional development of self and others, mentoring staff and offering supervision to a small number of OTs/ OTAs where this is agreed by the relevant Cluster Manager
  • For those OTs who are at Grade 6 above the bar, to also facilitate monthly group professional supervision of the OTs in the Area
  • To provide specific advice to colleagues regarding criteria for equipment via Fast Track
  • To contribute to the identification of assessors training needs including training for Trusted Assessor and similar courses
  • To work in partnership with and providing professional consultation, and supervision to other professionals, agencies and specialist groups including District Councils, Housing Associations, Community Care Practitioners and other peers
  • To assess for specialist equipment with suppliers
  • To carry out follow up contact advising on progress, outcomes and closures and providing relevant and timely information
  • To work towards internal and external performance targets
  • To deal with urgent requests from carers to identify and resolve safe moving and handling issues, and accurately record the outcomes
  • To accurately complete and maintain all appropriate records, ensuring confidentiality
  • To maintain and develop clinical expertise which is essential for professional registration and best practice
