Main purpose of the job

The post holder will be responsible for supporting the Contract and Service Development Managers and the wider ACS management team to reduce demand where appropriate, implement cost savings and deliver improved outcomes for customers.

The focus of the role is to engage with providers, customers and carers and other stakeholders to achieve the best quality Homecare or Residential and Nursing Home placement.

The post holder will develop innovative ways which facilitate and promote the full involvement of customers and their families in decision making and service reviews.

Success will be measured by the number of people who are placed, those who achieve the best possible outcomes from their placement and the ongoing demand management and value for money.

The post holder will represent Strategic Planning and Resources in all other areas of the business and with external partners to embed the health and care culture.
